PO-04-222 OUTFLOW TRACT VERSUS NON-OUTFLOW TRACT VENTRICULAR ARRHYTHMIAS IN ATHLETES: MYOCARDIAL SUBSTRATE AND LONG-TERM CLINICAL OUTCOMES

چکیده
Outflow tract ventricular arrhythmias (OT-VA) are common in healthy subjects without structural heart disease (SHD), and generally considered benign. However, athlete-specific data on the clinical meaning of different arrhythmia (VA) morphologies scarce. To assess prevalence SHD as well long-term sports medicine outcomes a cohort athletes with OT-VA undergoing comprehensive diagnostic assessment, by comparing them non-outflow VAs (nOT-VA). We conducted multi-center, observational, retrospective study enrolling who were referred because complex (>500 PVCs/24 h, exercise-induced VAs, nonsustained or sustained tachycardia). Athletes categorized into two groups: OT-VA, case monomorphic left bundle branch block inferior axis morphology; nOT-VA, polymorphic right superior/intermediate axis. The co-primary survival free from all-cause death practice at last follow-up. 175 (OT-VA,n=82; nOT-VA,n=93) included present analysis; characteristics resumed Table. Remarkably, signs was higher nOT-VA compared to (Table). A final diagnosis idiopathic VA more and, accordingly, catheter ablation commonly performed former group. Over median follow-up 61(43-85) months, there 2 cardiac arrests 1 appropriate ICD shock, all occurring group vivo evidence for an overall primary outcome event 0 3% groups, respectively (p=0.25). At follow-up, 31 (18%) practiced competitive (OT-VA vs. nOT-VA,p=0.23), while 92 (53%) leisure-time nOT-VA,p=0.91). Our confirm that underlying substrate is lower than major adverse events occurred VAs. Nevertheless, workup liberal use magnetic resonance allowed identification substantial minority clear implications eligibility assessment.

Clinical and electrophysiological spectrum of idiopathic ventricular outflow tract arrhythmias.
OBJECTIVES This study sought to compare and contrast the clinical and electrophysiological characteristics of outflow tract arrhythmias. BACKGROUND Idiopathic ventricular outflow tract arrhythmias manifest clinically in 3 forms: 1) paroxysmal sustained monomorphic ventricular tachycardia (SMVT), 2) repetitive nonsustained ventricular tachycardia (NSVT), or 3) premature ventricular contraction...
متن کاملRadiofrequency Catheter Ablation of Idiopathic Right Ventricular Outflow Tract Arrhythmias
Idiopathic ventricular arrhythmias (VA) consist of various subtypes of VA that occur in the absence of clinically apparent structural heart disease. Affected patients account for approximately 10% of all patients referred for evaluation of ventricular tachycardia (VT). Arrhythmias arising from the outflow tract (OT) are the most common subtype of idiopathic VA and more than 70-80% of idiopathic...

Although the pathogenesis of ventricular outflow tract tachycardia has not been fully elucidated, recent findings suggest that defects in cAMP signalling may be involved.
متن کاملLeft Ventricular Outflow Tract Obstruction
Patients with hypertrophic cardiomyopathy (i.e., asymmetric septal hypertrophy) may show obstruction to left ventricular outflow under basal conditions or with provocative maneuvers. The presence of dynamic left ventricular outflow tract obstruction in patients with concentric ventricular wall thickening (but without abnormalities of the aortic valve) has been less well appreciated. Clinical an...
متن کاملLeft Ventricular Outflow Tract Tachycardias
Left ventricular outflow tract tachycardia (LVOT-VT) is an uncommon type of idiopathic left ventricular tachycardia (ILVT) thought to be due to cyclic adenosine mono phosphate (cAMP) mediated triggered activity. The arrhythmia can be terminated pharmacologically with calcium channel blockers or beta-blockers [1]. LVOT-VT can arise above or below the coronary cusps and the former variety can als...
